HMH-Mountainside is seeking a full time Physician Advisor who would be responsible for managing the efficiency of care delivered at the organization and would report directly to the Vice President of Clinical Effectiveness and Medical Affairs (VPCEMA). The Physician Advisor will work with medical and nursing staff leadership of the hospital and will closely collaborate with the Director of Case Management and Social Work. He/She will ensure proper discharge planning and utilization management by developing and implementing methods to optimize use of hospital services for all patients while collaborating with the VP of Clinical Effectiveness and Medical Affairs to ensure the quality of the care provided. This includes working with those developing case management and utilization management protocols, working with physicians and others to reduce length of stay and ensuring patients are placed at the appropriate levels of care, and monitoring the appropriate use of diagnostic and therapeutic modalities to provide the most effective and efficient care that will result in the highest level of outcomes. The Physician Advisor is also responsible for assisting the organization in communicating and clarifying the numerous rules and regulations impacting resource management and quality goals. REPORTING RELATIONSHIPS: The Physician Advisor will report directly to the Vice President of Clinical Effectiveness and Medical Affairs with an indirect reporting relationship to the Chief Financial Officer RESPONSIBILITIES Essential duties and responsibilities include shown below: CLINICAL EFFECTIVENESS: Maintain accountability for achieving case management outcomes and fulfilling the obligations and responsibilities in support of the medical staff providing efficient and cost effective care for their patients Review issues identified by case managers to ensure appropriate follow-up, recommend improvement initiatives as needed, and make referrals to the appropriate areas of the hospital as warranted. Uphold the organization’s values of teamwork, interacting with others with dignity and respect and professionalism Provide consultation to nurses and case management staff and physicians regarding complex clinical issues and advising as to appropriate steps in the care and work flow process REVENUE CYCLE: Work collaboratively with the hospital leadership and the VPCEMA to ensure maximal performance on the hospital-based performance measures Participate in appeal and denial processes – evaluation of processes, source of denials and reason to respond to payers which would include peer-to-peer discussion and preparation of substantiating information to support and submit charges and rationale for the care process under review Review of denial causation, implementation of corrective actions and tracking of effects of changes in care models Participate in Medicare RAC reviews Participate in educational events emphasizing improvement in clinical documentation Evaluate gaps and opportunities to effectively educate the physicians and other staff on proper documentation needed to ensure proper reimbursement for services rendered Serve in a consultative role to finance, billing and coding on issues pertaining to clinical documentation, levels of service, etc. Work diligently to reduce length of stay and unnecessary admissions COLLABORATION: Facilitate internal and external relationships with all physicians and constituents of case management Work with the VP of Clinical Effectiveness and Medical Affairs to create strategies to enhance hospital and post-acute interdisciplinary efforts for maximizing patient/family outcomes. Serve as an advocate for and active participant in case management services Participate in developing and maintaining positive relationships with third party payers; ensure processes are followed according to policy and contracts related to third party payers Demonstrate a high level of excellent customer service, communication and interpersonal skills UTILIZATION MANAGEMENT: Communicate with the payers, medical directors and other physician advisors as requested to resolve any disputes surrounding medical necessity approvals for continued stay Participate in level of care determinations on a case-by-case basis Authorize all appeal letters via signature in conjunction with case management personnel Co-chair the Utilization Review Committee with other medical staff members and/or actively participate in all clinical utilization review activities PHYSICIAN LIASION: Collaborate with VPCEMA to conduct physician education sessions relevant to sharing of data, trends, practice patterns and other relevant information to assist in delivering effective, efficient and highest level of outcome care impacted by CMS rules and regulations Promote physician accountability for efficient patient care management Participate in coaching and educating residents and staff physicians on the appropriateness of level of care and timely need for discharge planning Investigate “avoidable delays” requests referred by case management Contact physicians to resolve delays and achieve positive outcomes Proactively identify denial trends and work with the medical staff and hospital executive team to resolve these issues OUTCOMES AND DELIVERABLES: • Develop dashboards and a monthly record of performance to be reviewed by the VP of Clinical Effectiveness and Medical Affairs; consider quarterly or regularly scheduled education sessions for medical staff on trends, practice patterns or relevant information referable to such • Develop written articles or an organizational newsletter on timely case management and resource management issues EDUCATION: Doctor of medicine or osteopathy with completion of specialty residency and board certification in internal medicine, family practice or emergency medicine. Surgical or other subspecialization will also be considered with appropriate experience referable to this position LICENSURE, CERTIFICATION AND REGISTRATION: License to practice medicine as a physician in the state of New Jersey EXPERIENCE: Preferred minimum of 10 years of clinical practice with a minimum of 3 years of managed care, quality management, utilization review, resource management at the payer or provider (hospital) level. Prior experience as a physician advisor would be advantageous and relevant continuing education programs pertinent to this position is highly recommended. As stated, this is a full time position but consideration would be given to a properly-trained clinician who also desires some part time practice.
